Those people signing as reviewers of individual test sections have the appropriate expertise, determined that the results are acceptable or that no further action is necessary on the given test, and that the test form has been properly completed.  These requirements also pertain to standard test procedures and standard test forms.
Those people signing as reviewers of the Installation Qualification (IQ) and Operational Qualification (OQ) summary pages have the appropriate expertise, reviewed the results of each test and determined that the system installation and operation is acceptable.  Also, persons signing as reviewers have reviewed the summary write-up section to verify that the pertinent issues have been addressed accurately and completely.
Those people signing as approvers of the final Qualification Package have the appropriate expertise, verified that reviewers have appropriate technical expertise, reviewed the summary write-up section, determined that the IQ/OQ is acceptable, and that the system is in a state of functionality as described in the functional specification.
